Most of these repairs will apply to all Generation 3 Plymouth, Dodge, and Chrysler minivans. So the key words are Gen 3, Gen III, Gen Three, Generation Three. Generation III Chrysler, Dodge, and Plymouth mini-vans were built in 1996, 1997, 1998, 1999, and 2000.
Now that the "single" headlight assemblies are only about $50 complete with the headlight bulb installed (but not the turn signal bulb) and the "quad" headlights are only about $60 with two new bulbs (the quad doesn't have the turn signal), it makes sense to change them yourself.
A bolt at the top and three nuts in the back are all that hold the headlight on. Access to the lower nuts is a little tight, but really not bad. This video and some photos will show some details.
Here is another video describing the procedure involved with the quad headlights for the Chrysler Town and Country. They're basically the same, but it might have some extra information that you'll find helpful.
